Aviv adds geriatric care team

NORTH SHORE - Aviv Centers for Living has added Geriatric Care Management to its continuum of care, which includes short- and long-term care, memory care, home care, adult day services and assisted living. Geriatric care managers can complete an assessment of an older adult's current level of functioning and examine his or her support systems and resources.
